In connection with the Personal Data Protection Act of 29 August 1997 (Journal of Laws of 2016, item 922, as amended), I hereby authorise the processing of my personal data for the purpose of replying to my enquiry. At the same time, I acknowledge that in relation to the personal data I have provided to the CCF, I shall hold the rights specified in Article 32 of the Personal Data Protection Act of 29 August 1997 (Journal of Laws of 2016, item 922, as amended), in particular the right to access and correct the data and the right to request that its processing be discontinued.
